Udok sick notes include a QR code for verification. Scan the code or use the verification page to confirm the note is valid.


Option 1: Scan the QR Code

  • Open your phone camera and scan the QR code printed on the sick note.
  • The verification page will open and show whether the sick note is valid.

Option 2: Use the Verification Page

  • Open the verification page and paste the token from the sick note into the URL.
  • Example format:
https://app.udok.co.za/verify/sick-note?token=YOUR_TOKEN

What You Will See

If the sick note is valid, the page shows:

  • Issued date and time
  • Patient name and date of birth
  • Doctor name and practitioner number
  • Practice details
  • Consultation and document IDs

If the sick note cannot be verified, the page will show an error message.
